What Structured Family Caregiving means
Structured Family Caregiving, often called SFC, is a home- and community-based service that may support an eligible person who lives with a qualified caregiver. The caregiver provides daily help, supervision, and household support in accordance with an approved person-centered service plan. A provider agency supplies oversight, coaching, care coordination, and required documentation.
Who may want to explore SFC?
Indiana PathWays for Aging generally serves eligible Hoosiers age 60 and older who receive Medicaid, with or without Medicare. Structured Family Caregiving may be considered when the individual needs daily support and lives with the family member or other qualified caregiver providing that care.
