Indiana PathWays for Aging
For qualifying Indiana Medicaid members age 60 and older, PathWays may include home- and community-based supports such as attendant care, respite and Structured Family Caregiving.

This arrangement may allow a qualified live-in family or nonfamily caregiver to provide daily support, receive training and receive a program-approved per-diem stipend.

Structured Family Caregiving is designed for an eligible participant who lives with a primary caregiver. The caregiver provides daily care based on assessed needs and receives training and ongoing support through an approved provider agency.
- The participant generally must meet Medicaid and functional eligibility requirements.
- The participant and caregiver must live together in an approved private-home setting.
- A service coordinator and provider agency help establish and oversee the care plan.
- Final eligibility, service level and authorization come from Indiana Medicaid and the member’s PathWays health plan.

How to begin with VA services
- Enroll in VA health care if you are not already enrolled.
- Speak with your VA primary-care team or social worker about home-care needs.
- Request an assessment for available home- and community-based services.
- If services are authorized and White Oak is an available approved provider, ask whether White Oak Home Care may serve you.
VA-supported care at home
VA programs can help some Veterans with bathing, dressing, meal preparation, mobility and other daily activities. Clinical eligibility and local program availability apply; authorization must come from the VA.
