Originally rooted in Whiting and now based in Crown Point, we serve families throughout Northwest Indiana. Our local team understands the communities, healthcare networks and practical challenges families face when a loved one needs help at home.

Helping families understand possible funding paths
White Oak serves private-pay clients and helps eligible families explore authorized home-care options through Indiana Medicaid programs and the Department of Veterans Affairs.
Indiana PathWays
Qualifying Indiana Medicaid members age 60 and older may have access to home- and community-based services, including attendant care, respite and Structured Family Caregiving.
Structured Family Caregiving
Under an authorized arrangement, a qualified caregiver who lives with the participant may provide daily support, receive training and receive a program-approved stipend.
Veterans care
Eligible Veterans with an assessed clinical need may qualify for VA-authorized home-care services, depending on local availability and approval.
Program eligibility, authorization and covered services are determined by the applicable government program or health plan.
