What is Indiana PathWays for Aging?

Indiana PathWays for Aging is the state Medicaid program for many Hoosiers age 60 and older who receive Medicaid, or both Medicaid and Medicare. Participants choose a health plan that coordinates medical care and long-term services and supports. Depending on an individual assessment and approved service plan, supports may include help with meals, personal care, transportation, home health visits, adult day services, and other home- and community-based services.

Important: Indiana Medicaid and the member’s PathWays health plan determine eligibility, covered services, provider choice, and authorized hours. White Oak Home Care cannot promise enrollment, authorization, or payment.

How the process generally works

Confirm Medicaid eligibility

PathWays generally serves Medicaid-eligible adults age 60 and older, including qualifying people receiving long-term services and supports at home or in a community setting.

Choose or confirm a health plan

Indiana currently lists Anthem, Humana, and UnitedHealthcare as PathWays health plans. Your plan can explain benefits, provider networks, and care coordination.

Complete an assessment

A care coordinator evaluates daily needs, safety concerns, caregiver support, and the level of assistance that may be appropriate.

Review the approved care plan

Services begin only after they are included in an approved plan and an eligible provider is selected.

Ways in-home assistance may help

Personal support

Assistance with bathing, grooming, dressing, toileting, mobility, and other daily routines when authorized.

Home routines

Meal preparation, light housekeeping, laundry, medication reminders, and safety-focused support.

Caregiver relief

Respite and dependable scheduled assistance can help family caregivers maintain balance and continuity.
